My guess on how Sarada awakens it is that she reaches a stagnation point in her training and after a lot of consideration, Uchiha Sasuke puts her in a Tsukuyomi where she accidentally kills genjutsu versions Uzumaki Boruto and Mitsuki, awakening her Mangekyo Sharringan without having to hurt her loved ones, and afterwards Sasuke tells her to only use it when absolutely necessary. And after she starts going blind, Uchiha Sasuke goes to Orochimaru and he utilizes stem cells from an egg from Sakura and Uchiha Sasuke to make a new pair of Uchiha eye’s and enjects them with hormones to mature them to Mangekyo, and then has Sakura transplant them to Sarada (her daughter).
